The intake process must be completed before we are able to schedule an appointment. The steps of the intake process consist of answering the initial questions on our new patient inquiry form, as well as completing the paperwork and insurance steps. Once we receive your answers to our initial questions, we will get you set up in our system. We will then send you an email with two required forms: an Intake Form your provider will review and an Office Policies and Patient Agreement Form for you to sign. The email will also provide you with instructions to send us a copy of your ID, and both the front and back of your insurance card. After we have received your completed forms, ID and insurance card, we will verify your benefit details and reach out to offer you an appointment.
We do not do formal academic, psychological, or neuropsychiatric evaluations that lead to an extensive multi-page report that may be required for court, schools, or disability assessments. However, our providers will document an encounter, including symptoms and diagnoses, as well as recommended treatments or further evaluations. Our practice focus is on treatment, rather than formal report preparation.
